Job summary

Mattapan Health & Rehabilitation Center is seeking a compassionate Speech-Language Pathologist to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide skilled therapy addressing communication, cognition, and swallowing needs.

Join an interdisciplinary team and help residents regain independence and quality of life in a resident-centered environment. Massachusetts-licensed SLP with CCC-SLP preferred.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Massachusetts license to practice as a Speech-Language Pathologist.
• CCC-SLP certification from ASHA preferred.
• Experience in skilled nursing, long-term care, rehabilitation, or related healthcare setting preferred.
• Strong clinical reasoning, evaluation, and treatment planning skills.
• Knowledge of regulatory requirements and excellent communication skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans to improve communication, cognition, swallowing, safety, and independence.
• Provide skilled speech-language pathology services per physician orders and plans of care.
• Assess swallowing and feeding difficulties and implement safe, effective interventions.
• Collaborate with nursing, physicians, and other therapists as part of an interdisciplinary team.
• Maintain accurate clinical documentation in compliance with state and federal regulations.
• Support discharge planning and participate in resident care planning meetings.
