Job summary

Mercy Medical Center Home Health and Hospice is seeking a licensed Speech-Language Pathologist to cover a Canton, OH territory. The role involves providing skilled speech therapy and documenting care while collaborating with physicians and care teams.

A current SLP license is required; one year of experience is preferred, and home care experience is desirable. Travel and a valid driver's license are required; benefits include medical, dental, vision, PTO and 401k options.

Qualifications

• A current Speech-Language Pathologist (SLP) license in the state of employment.
• Minimum of one year of experience as an SLP is preferred.
• Home care experience is desired.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled speech therapy as ordered by the attending physician.
• Evaluate patient functioning and develop an individualized plan of care.
• Document treatment, assessments and education in the chart.
• Coordinate with care team for admissions, transfers and discharges.
• Maintain professional appearance and adhere to agency policies.
