• Provides direct patient care meeting standards, including evaluation, care plan, treatment, and discharge planning; inpatient/outpatient.
• Designs and modifies interventions to address swallowing/speech and language impairments and functional limitations.
• Documents assessment, plan of care, progress notes, and discharge summaries; notes patient and family participation.
• Discharges customers with discharge planning and family training; informs Recovery Care Coordinator.
• Schedules and maintains caseload consistent with productivity standards.
• Alerts supervisor to problems with delivery of care or caseload; ensures timely, accurate billing.
• Participates in interdisciplinary care planning, rounds, and care conferences; coordinates with community agencies.
• Identifies equipment needs and makes recommendations for repairs, upgrades, purchases.
• Attends and participates in in-service training.
