Job summary

Pascua Yaqui Tribe seeks a Social Worker I to manage a caseload within a specified social services program. You will determine benefits per regulations, perform case management tasks, educate clients, make referrals, and monitor client progress.

The role emphasizes assessments, referrals, documentation, and collaboration with tribal departments and outside agencies to ensure program compliance and quality client care.

Qualifications

• Case Manager with three years of experience and degrees as listed, or equivalent experience.
• Bachelor's/Associates degree in relevant fields or equivalent experience.
• Proficiency with MS Word and Outlook and knowledge of regulations.

Responsibilities

• Provide assessments of community members and referrals for resources.
• Assist clients in filling forms and verifying information for programs.
• Maintain regular client contact via visits and calls; monitor status and progress.
• Develop service plans and document progress; update plans as needed.
• Coordinate with tribal departments and outside agencies to ensure compliance.
