Qualifications

• Bachelor’s degree in Social Work or related field required.

• LBSW or LMSW encouraged to apply.

Responsibilities

• Lead and coordinate discharge planning to ensure safe and timely transitions.
• Provide psychosocial support to residents and families.
• Collaborate with nursing, therapy, and leadership on care plans and care conferences.
• Support admissions, transitions, and resident assessments.
• Connect residents and families with community resources and services.
• Ensure compliance with state and federal regulations, including Centers for Medicare & Medicaid Services guidelines.
• Maintain accurate, timely, and compliant documentation.
