Job summary

TriStar Centennial Medical Center in Nashville, TN seeks an experienced Social Worker (MSW) to perform biopsychosocial assessments, crisis intervention, and discharge planning on inpatient floors and the ED.

You will collaborate with RN case managers, physicians, and insurance staff to address barriers to discharge, provide emotional support, and coordinate post-acute care tailored to patient and family needs.

Qualifications

• MSW required or equivalent
• Experience with inpatient and ED social work
• Strong knowledge of discharge planning and post-discharge planning

Responsibilities

• Perform biopsychosocial assessments for inpatients and ED patients
• Provide crisis intervention and counseling to patients and families
• Coordinate discharge planning and post-acute care placement
• Collaborate with RN case managers, physicians, and insurance staff
• Assist with end-of-life or complex emotional support scenarios
