Qualifications

• Bachelor of Social Work required; MSW preferred.
• LBSW, LCSW, or MSW licensure preferred.
• Minimum 2 years hospital or multidisciplinary healthcare setting experience.
• Strong knowledge of community resources and crisis intervention.
• Good communication and documentation skills.

Responsabilités

• Complete psychosocial assessments and care coordination.
• Provide crisis intervention and counseling.
• Engage patient and family in discharge planning and post-acute planning.
• Educate patients and team about psychosocial factors impacting illness.
• Ensure compliance with regulatory reporting and documentation.
