Job summary

Qualifications

• Master's degree in Social Work from an accredited college or university.
• Two or more years’ experience working with the patient population preferred.

Responsibilities

• Perform psychosocial assessments and develop treatment plans while ensuring safety.
• Gather information via observation and interviews to tailor discharge plans.
• Provide crisis intervention and help families understand implications of life events.
• Join multidisciplinary teams to discuss assessments, planning and discharge.
• Coordinate cases across illness, hospitalization, finances, education and services.
• Facilitate education, process groups, support groups and referrals as requested.
• May conduct intakes and assist in coordinating placements within programs.
• Promote recovery, symptom reduction and maximal social functioning.
• Support families and communicate care needs to them.
• Maintain caseload and timely patient chart documentation.
