Qualifications

• Knowledge of discharge planning, crisis intervention and community services.
• Ability to prioritize multiple tasks and work with a care team.

Responsibilities

• Intervenes with patients/families for psychosocial needs and assists with discharge planning.
• Coordinate referrals to community resources and crisis intervention as needed.
• Collaborate with interdisciplinary team to develop and implement discharge plans.
• Provide inpatient emotional support and bereavement services when appropriate.
