Qualifications

• Masters in Social Work (MSW) required.

• 0-1 years of social work experience in hospital or related health care setting, Emergency Department experience preferred.

Responsibilities

• Intervenes with patients/families for psychosocial needs and provides crisis intervention.
• Coordinates and facilitates development of discharge plans of care; advocates for patient/family needs.
• Provides patient/family services via case finding referrals from physicians and healthcare team members; liaises with community resources.
• May provide inpatient emotional support and counseling, bereavement services, and support for abuse/neglect cases.
