Qualifications

• MSW required or equivalent education for case management.
• 0–1 year in hospital/social work settings preferred; internship counts.
• Strong communication and negotiation abilities essential.
• Solid data management and computer skills.
• Knowledge of discharge planning and crisis intervention is preferred.

Responsibilities

• Intervenes with patients/families for psychosocial needs and coping.
• Provide discharge planning, education, counseling, advocacy, and referrals.
• Coordinate crisis intervention and discharge plans with the care team.
• Serve as liaison between patients and community resources.
• Identify psychosocial and environmental needs related to admission and discharge.
• Collaborate with interdisciplinary team on discharge planning.
• Make referrals to community services as needed.
• Provide inpatient emotional support and bereavement services, if needed.
• Address child or elder abuse/neglect and domestic violence concerns.
