Responsibilities

• Performs resident assessments and contributes to admission/process reviews.
• Creates, reviews, and updates care plans and progress notes.
• Coordinates visits with outside services (dental, optical, etc.).
• Attends and documents resident council meetings.
• Assists families in coping with skilled nursing placement and grief.
• Plans discharge with patient, family and team members.
• Conducts in-service training for staff on psychosocial issues and patient rights.
• Supervises Social Services Assistants.
• Performs other tasks as assigned.
