Job summary

TCRC is seeking a Service Coordinator II in Fillmore to provide person-centered consultation, advocacy, and service coordination for individuals with developmental disabilities needing more specialized assistance.

You will support IFSP/IPP processes, attend IDT meetings, and connect clients with schools, day programs, and local agencies. A strong emphasis on collaboration and ethical practice guides every interaction.

Qualifications

• Bachelor’s degree in Human Services or related field required.
• Minimum of 2 years of social and/or mental health experience.
• Valid California driver’s license required for this position.
• Experience working with individuals with developmental disabilities preferred but not required.

Responsibilities

• Support individuals by maintaining regular contact and creating IFSP/IPP.
• Participate in IDT staffings and community meetings (IEP/day program).
• Provide information and referrals to community services (schools, employment programs, agencies).
• Coordinate and secure TCRC funded services (behavioral services, respite, day programs).
• Develop and maintain relationships with vendors and community organizations.
