Job summary

Arab-American Family Support Center in New York City is seeking a Solution-Based Casework (SBC) Case Planner for a full-time hybrid role in the Bronx. The position reports to the SBC Supervisor and offers a salary of $62,197.89 annually.

The Case Planner will provide intensive preventive services, conduct home and office visits, develop service plans, and coordinate with ACS and community resources to support at-risk families and prevent foster care placements.

Qualifications

• Bachelor’s degree from an accredited college or university required.
• Familiarity with PROMIS and Connections.
• Computer skills in Microsoft Office suite and excellent writing skills are required.
• Bilingual or multi-lingual fluency required; Arabic, Bangla/Bengali, Hindi, Spanish or Urdu strongly preferred.

Responsibilities

• Provide high-quality case planning, case work, and case management services to at-risk families referred by ACS.
• Complete and submit intake paperwork and maintain case files.
• Conduct home visits and office visits to assess child safety and risk elements.
• Advocate for families by linking them to needed services in the community.
• Coordinate monthly visits with relevant ACS and community partners.
• Attend trainings and maintain SBC certifications as required.
