Job summary

Centre for Neuro Skills is seeking a Clinical Case Manager in Irving, TX. This role oversees development and management of all clinical aspects, coordinating care across clinic and residential sites, and collaborating with the Director of Rehabilitation.

You will collect patient information, plan actions, coordinate with families and external partners, implement interventions, and evaluate outcomes to drive independence and quality of life for patients with traumatic brain injury.

Qualifications

• Bachelor’s degree in a related field is required.
• Master’s or licensure/credential preferred but experience can offset this level of education/licensing.
• CCM certification is preferred; CCM completion may be required to meet eligibility guidelines.

Responsibilities

• Develop, implement and manage clinical case management plans.
• Coordinate with staff to ensure care objectives are met.
• Communicate with patients’ families and external stakeholders.
• Oversee plan evaluation and adjust as needed to achieve goals.
• Maintain documentation of processes across clinic and residential sites.
