Job summary

People Well Staffing seeks a Physical Therapist Assistant to join a skilled nursing facility rehabilitation team in Santa Rosa, CA. You will work under a licensed Physical Therapist to carry out individualized treatment plans and help patients progress toward functional goals.

The role requires graduation from an accredited PTA program, a CA PTA license (or eligibility), BLS/CPR, and strong patient-care and teamwork skills. Prior SNF experience is preferred.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active CA PTA license in good standing, or eligibility for licensure.
• Current BLS/CPR certification as required by the employer.
• Strong communication, documentation, teamwork, and patient-care skills.
• Experience in the applicable SNF setting preferred.

Responsibilities

• Implement treatment plans established by the supervising Physical Therapist.
• Provide therapeutic exercise, gait, balance, mobility, and functional training.
• Monitor patient response and communicate progress to the Physical Therapist.
• Educate patients and caregivers within the established plan of care.
• Complete accurate and timely documentation.
• Collaborate with the rehabilitation and interdisciplinary care team.
