Qualifications

• Must hold current Massachusetts LICSW license.
• Crisis intervention experience preferred.
• Experience in inpatient psychiatric hospital setting preferred.
• Ability to assess patients' psychosocial, emotional and financial needs.

Responsibilities

• Identify new admissions to the psychiatric unit and review for clinical appropriateness and treatment plan.
• Develop an individual plan of care and discharge plan in collaboration with patient/family and care team.
• Implement and monitor the plan of care with ongoing support and resources for patients and families.
• Effectively communicate discharge needs and obstacles to the interdisciplinary team.
• Maintain credentials in social work and participate in continuing education programs.
• Provide support and consultation to other health care team members.
