Job summary

PIH Health is seeking a Licensed Clinical Social Worker (LCSW) for ambulatory, part-time engagement. The role focuses on assessing psychosocial needs, coordinating care plans, and coordinating with community resources for patients and families.

The LCSW will also provide psycho-social therapy, participate in health-team conferences, and support the transition/discharge planning, in collaboration with the Director and medical teams.

Qualifications

• Master's Degree from an accredited School of Social Work.
• California licensure as LCSW required.
• Experience with regulatory reporting and continuing education.

Responsibilities

• Assess and intervene with patients and families to address psychosocial needs.
• Develop and implement care plans with the interdisciplinary team.
• Coordinate with community resources and referrals as needed.
• Provide psycho-social therapy and participate in team conferences.
