Qualifikationen

• Bachelor's Degree plus 1 year experience in mental health/psychiatric rehabilitation is required.
• AA degree with at least 2 years experience in mental health/psychiatric rehabilitation may be considered.
• Ability to work as part of a team and independently.
• Effective verbal and written communication skills are required.
• Good driving record is required.

Aufgaben

• Coordinate and provide program services (Residential, Psychiatric Rehabilitation Program, Health Home, community housing, external providers).
• Provide services in various locations (day program, home, community).
• Assist individuals with developing and carrying out long- and short-term goals and track progress.
• Schedule and accompany individuals to medical and behavioral health appointments.
• Complete required documentation in EHR such as progress notes, ISPs, Monthly Progress Notes, and annual documents.
• Communicate with Intake Coordinator, Residential Supervisor and Program Director to ensure linkage of resources and services.
• Ensure eligibility for SSA, DHS, VA entitlements and annual redeterminations.
• Connect to specialized services such as MTA, birth certificates, ID cards, Section 8 Housing.
• Coordinate with RRP and outside providers to ensure prescription refills.
• Engage in crisis management and participate in emergency on-call rotation.
• Facilitate meetings with rehabilitation team (PRP Counselor, SEP, HH).
• Utilize computer systems for communication.
• Engage in day program activities such as group facilitation, as needed.
• Provide monthly stats to PRP Director (hospitalizations, employment, etc.).
• Perform other duties as requested by program supervisors.
