Job summary

Advanced Nursing Concepts seeks a licensed physical therapist to provide in-home care with a focus on functional assessment and mobility rehabilitation. You will work with physicians to implement plans of care, educate patients and families, and coordinate needed home health services.

The role requires a current state license, CPR certification, and reliable vehicle for travel across the service area, with 2–3 days weekly availability and a PRN schedule.

Qualifications

• Degree in physical therapy (baccalaureate or Master’s) from an accredited program.
• Licensed to practice as a physical therapist in the state (where applicable).
• Two years of relevant PT experience; home health preferred.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Must travel regularly by personal vehicle within the service area.

Responsibilities

• Assess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including home environment.
• Carry out the physical therapy plan of care set by the physician.
• Instruct patients and families on therapeutic appliances and prosthetic devices.
• Document care accurately and on time, including clinical and progress summaries.
• Coordinate with physicians and report patient response to treatment or changes in condition.
• Identify needs for other home health services and refer as appropriate.
• Participate in discharge planning and case conferences.
• Provide consultation to families/caregivers and inservice education to organization personnel.
• Supervise physical therapy assistants per organization policy and state regulations.
• Check in on home health aide care every two weeks when therapy is the only skilled service.
• Take on other duties as assigned.
