Job summary

Advanced Nursing Concepts seeks a licensed physical therapist to evaluate patients in home health settings across Crystal River and surrounding areas. You will implement physician-approved plans of care, educate patients and families on therapeutic devices, and document progress accurately.

The role requires travel by personal vehicle, two years of home health experience, CPR certification, and strong communication and organizational skills.

Qualifications

• Degree in physical therapy (baccalaureate or master's) per accreditation body.
• Licensed to practice as a physical therapist in the state.
• Two years of clinical/house/home health experience preferred.
• Excellent verbal and written communication and organizational skills.
• Current CPR certification.
• Must have valid driver's license, reliable transportation, and ability to drive for extended periods.

Responsibilities

• Assess patients' functional status, muscle strength, mobility, gait, ROM, and rehab potential.
• Carry out the plan of care set by the physician.
• Instruct patients and families on therapeutic appliances and prosthetic devices.
• Document care accurately and timely, including progress summaries.
• Coordinate with physicians and report patient responses to treatment.
• Refer to other home health services when needed.
• Participate in discharge planning and case conferences.
• Provide consultation to families and inservice education to staff.
• Supervise physical therapy assistants per policy and state regulations.
• Monitor home health aide care and ensure quality of service.
