Job summary

Heart of Florida Health Center is seeking a physician to provide comprehensive outpatient primary care services. You will work closely with a multidisciplinary team to deliver preventive, acute, and chronic care, while adhering to HFHC policies and standards.

The role requires an MD/DO, active Florida medical license and DEA, BLS certification, and completion of residency if applicable. Strong documentation, communication, and commitment to quality care are essential.

Qualifications

• MD/DO degree from an accredited medical school.
• Active Florida State Medical License.
• Active DEA License.
• Board Certification or Board Eligibility in Family Medicine, Internal Medicine, General Practice, Pediatrics, or Women’s Health.
• Maintain CME credits to stay current.
• Complete HRSA credentialing requirements.

Responsibilities

• Provide comprehensive outpatient primary care services.
• Collaborate with a multidisciplinary team.
• Maintain documentation and coding in EMR per HFHC policies.
• Follow disease management protocols and clinical guidelines.
• Participate in quality assurance and peer review processes.
• Ensure timely communication with Medical Director/CMO as needed.
