Job summary

Community Health Centers in Florida seeks a dedicated physician to provide outpatient primary care services, including preventive care, acute and chronic management, and clinical evaluation. You will adhere to CHC policies, work with care teams, maintain documentation in EHR, participate in quality initiatives, and rotate to other centers as needed.

A DEA license and BLS certification are required, and residency in Family Practice is preferred.

Qualifications

• MD/DO with residency in family practice.
• 3–5 years medical experience preferred.
• DEA license and BLS certification required.

Responsibilities

• Adhere to CHC policies and state/federal regulations.
• Follow disease management protocols and clinical pathways.
• Complete documentation and timely tasks in EHR.
• Coordinate referrals and social services with care team.
• Participate in mandatory CHC meetings and quality assurance activities.
• Provide off-hour and Saturday clinic coverage as assigned.
