Job summary

LiveWell Homecare Agency, LLC is seeking a Nurse Practitioner to provide direct patient care within established protocols. You will assess and manage patients from six months to geriatric, interpret ECGs and lab results, and handle emergencies in a primary care setting.

The role requires 2-4 years in Family Medicine, valid NP/PA credentials with board certification, and BLS/ACLS. DEA registration if prescribing meds may be needed.

Qualifications

• 2-4 years’ experience, preferably in Family Medicine.
• Valid, unrestricted NP or PA license; board certification (FNP, AGNP, or PA-C).
• BLS and ACLS certifications.
• DEA registration if prescribing medications.

Responsibilities

• Conduct assessments, evaluations, and treatments for patients from six months to geriatric.
• Interpret ECG and lab results.
• Manage chronic illnesses such as hypertension, diabetes, and asthma.
• Promote disease prevention through risk assessment, coaching, and counseling.
• Complete Medicare Wellness physicals per policies and guidelines.
• Handle emergent medical situations effectively.
• Communicate professionally with patients and healthcare team.
• Perform duties at various primary care locations as needed.
