Eventus WholeHealth is seeking an Advanced Practice Clinician (NP or PA) to deliver comprehensive primary care within skilled nursing and senior living communities. The role emphasizes preventative care, chronic disease management, and coordination with interdisciplinary teams to improve outcomes for residents.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Excellent communication and teamwork to build relationships with residents, families, and staff.

Responsibilities

• Provide comprehensive primary care within skilled nursing and senior living communities.
• Perform routine rounds and proactive chronic disease management.
• Coordinate care across interdisciplinary teams and facility partners.
• Collaborate with physicians, behavioral health, care coordinators, and staff to deliver person-centered care plans.
