Qualifications

• Graduate from Specialty Training Program-Nursing (Required).
• Minimum of 1 year Nursing experience (Required).

Responsibilities

• Performs client assessment and triage including coordinating referrals to in-house clinical and community services.
• Provides primary clinical support to the provider, patient, and family to assist in diagnosis and treatment.
• Completes diagnostic follow-up to ensure accuracy and proper filing in medical records.
