Saber Health Group is seeking a Physical Therapist to join our skilled nursing facility team in Chester, VA. You will work Monday through Friday, helping residents regain and maintain physical function through targeted therapy plans.

You’ll evaluate patients, design exercise programs, and educate on independent home programs while collaborating with a caring, multidisciplinary team in a supportive environment.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited college or university.
• Active, unencumbered PT license for the state of employment.
• Basic computer skills and ability to use EHR and service delivery software.

Responsibilities

• Return patients to optimal physical function in SNF settings.
• Instruct patients on maintaining function after discharge and through independent exercises.
• Collaborate with therapy team to achieve patient and physician goals.
