Saber Healthcare Group is seeking a licensed Physical Therapist to join our skilled nursing facility therapy team. You will work to return residents to their optimal physical function, design individualized plans, and educate on maintenance post-discharge.

The position offers full-time, part-time, and PRN opportunities with shifts on weekdays and weekends. A bachelor’s degree or higher in Physical Therapy and an active state license are required, along with basic computer skills and EHR

Qualifications

• Bachelor’s degree or higher in Physical Therapy from an accredited program.
• Active, unencumbered Physical Therapy license in the state of employment.
• Basic computer skills and ability to navigate service delivery software and EHRs.

Responsibilities

• Return residents to optimal physical function and meet therapy goals in a SNF setting.
• Instruct and educate residents to maintain function after discharge through exercise and activity modification.
• Develop individualized treatment plans and document progress in the patient record.
