Qualifications

• Bachelor’s degree or higher in Physical Therapy from an accredited program.
• Must hold an active, unencumbered Physical Therapy license in the employment state.
• Proficient computer skills and ability to use service delivery software and the electronic health record software.

Responsibilities

• Restore patients to optimal physical function and meet therapy goals in a SNF setting.
• Educate patients on maintenance, independent exercises, and activity modification.
