Job summary

Hartford HealthCare in Bridgeport, CT seeks a Physical Therapist to evaluate, develop and implement individualized treatment plans across home care, outpatient, inpatient, skilled nursing, or assisted living settings, partnering with an interdisciplinary team to improve mobility and quality of life.

You will deliver evidence-based care with autonomy, mentor PTAs and students, and contribute to a culture of integrity, caring, excellence, and safety.

Qualifications

• PT experience required.
• Graduate of Physical Therapy Program from an APTA-accredited school; BS or entry-level master's degree.
• Active CT PT license.
• CPR certification required at time of start.

Responsibilities

• Evaluate patients' functional abilities, mobility, strength, balance, coordination, gait, endurance, and pain levels.
• Develop individualized treatment plans to maximize movement, safety, independence.
• Deliver evidence-based interventions, including exercise, manual techniques, and therapeutic modalities.
• Monitor progress and modify treatment plans to ensure optimal outcomes.
• Educate patients and caregivers on treatment plans, precautions, equipment, and strategies for continued success.
• Collaborate with physicians, nurses, social workers, rehabilitation professionals, and other members of the healthcare team.
• Support smooth transitions of care and discharge planning.
• Maintain timely and accurate clinical documentation.
• Serve as a mentor and clinical resource for Physical Therapist Assistants, students, and support staff as appropriate.
