Hartford HealthCare’s outpatient rehabilitation team in Windham, CT seeks a Physical Therapist to restore function, improve mobility, and reduce pain. You’ll work in a collaborative, multidisciplinary environment with diverse clinical cases across orthopedics, neurology, and functional conditions, helping patients return to daily activities.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Connecticut Physical Therapist license.
• Strong communication, interpersonal, and clinical skills.
• Passion for patient-centered care and helping individuals achieve their rehabilitation goals.
• Ability to collaborate effectively within a multidisciplinary team.

Responsibilities

• Perform comprehensive patient evaluations and design individualized treatment plans.
• Deliver evidence-based physical therapy interventions tailored to each patient.
• Treat a diverse patient population with orthopedic, neurological, and other conditions.
• Collaborate with occupational therapists, SLPs, physicians, and other health professionals to coordinate care.
• Educate patients and families on treatment plans, home exercises, and long-term strategies.
• Monitor progress and adjust treatment approaches as needed.
• Maintain accurate clinical documentation in a timely manner.
• Contribute to a positive, supportive rehabilitation environment.

What You’ll Do

As a Physical Therapist, you'll play an essential role in helping patients restore function, improve mobility, reduce pain, and return to the activities they enjoy.

Your Responsibilities Will Include

• Conducting comprehensive patient evaluations and developing individualized treatment plans.
• Delivering evidence-based physical therapy interventions tailored to each patient’s needs and goals.
• Treating a diverse patient population with a variety of orthopedic, neurological, and other functional conditions.
• Collaborating with occupational therapists, speech-language pathologists, physicians, and other healthcare professionals to coordinate patient care.
• Educating patients and families on treatment plans, home exercise programs, and strategies to support long-term success.
• Monitoring patient progress and modifying treatment approaches as clinically appropriate.
• Maintaining accurate and timely clinical documentation.
• Contributing to a positive, supportive, and patient-centered rehabilitation environment.
