Job summary

Nans Pointe Rehabilitation and Nursing is seeking a Physical Therapist to assess and treat patients in our geriatric rehabilitation program. You will provide direct patient care, establish treatment plans, and work under the direction of the Physician to promote recovery and function.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid professional license and registration for state of practice.
• Experience in long-term care or geriatric settings preferred.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care and evaluate medical condition and functional status.
• Develop and administer treatment plans with physician guidance.
• Set realistic goals based on patient capacity.
• Document patient conditions and educate patients and families.
• Complete evaluations, weekly progress notes, recertifications, and discharge documents.
• Collaborate with all disciplines for patient-centered care.
• Perform other duties as assigned by Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.
