Responsibilities

• Evaluates, plans and implements a goal oriented Physical Therapy treatment program that focuses on restoring function and preventing further disability.
• Delivery of care and maintaining a safe environment while delivering services.
• Discharge planning and timely documentation of progress and outcomes.
• Gathers information necessary to evaluate resident needs including testing and consultations with other health professionals.
• Maintains appropriate patient education and communicates effectively with residents and families.
