Job summary

Qualifications

• Graduate of an APTA accredited program or equivalent.
• Physical Therapist License.
• BLS HCP Certification.
• Excellent written and verbal communication skills.
• Proficient computer skills (MS Office/Google Suite).

Responsibilities

• Evaluates, plans and implements a goal oriented Physical Therapy treatment program that focuses on restoring function and preventing further disability.
• Delivery of care with emphasis on safety and proper body mechanics; conducts resident treatments as scheduled.
• Discharge planning and documentation including progress notes and discharge summaries.
• DME assessment and education on wheelchair fitting and use.
• Develops individualized treatment plans and maintains timely progress documentation.
• Provides patient and family education and communicates effectively with the care team.
