Job summary

Hackensack Meridian Health is seeking a Physical Therapist to organize and conduct medically prescribed physical therapy programs to restore function and prevent disability. You will evaluate patients, plan therapy with exercises and modalities, and document progress while collaborating with physicians, nursing, social services, and discharge planners.

The role requires a PT license, a DPT/MS/BS-PT degree, and current BLS.

Qualifications

• Bachelor of Science in Physical Therapy, MS in Physical Therapy or Doctorate in Physical Therapy (DPT).
• Licenses: Physical Therapist License.
• BLS/ACLS or similar Basic Life Support certification.

Responsibilities

• Performs physical therapy assessments and evaluations and administers treatments.
• Plans therapy involving exercises, massage, heat, water, etc.
• Evaluates, records and reports patient progress.
• Coordinates with care team for discharge planning and continuation of therapy.
• Attends inservice programs and contributes to quality improvement in PT.
