Job summary

Logan Health in Montana is seeking a physical therapist to plan and implement individualized treatment programs based on evaluation and ongoing assessment. You will develop an updated plan of care, set goals, and coordinate care with the team to support patient progress.

The role requires Montana PT licensure (or a multi-state compact license), BLS within seven days, strong communication and organizational skills, and the ability to work both in a team and independently.

Qualifications

• Montana PT license or multi-state compact license required.
• Graduate of accredited PT program and license within 30 days of hire.
• RQI Provider (BLS) within seven days of hire.

Responsibilities

• Develops, implements, and revises individualized physical therapy plans through evaluation and ongoing assessment.
• Performs comprehensive assessments reflecting patient status across functional domains.
• Maintains an updated plan of care with goals, interventions, and visit frequency.
• Plans, communicates, supervises, and delegates therapy services effectively.
• Performs treatments per PT standards and department policies.
• Completes admissions, discharge evaluations, and daily assessments with patient goals.
• Exhibits excellent customer service and communication to internal/external customers.
• Documents timely according to payer guidelines and Montana practice guidelines.
• Participates in department/community projects to enhance rehab services.
• Maintains productivity as per department protocol.
