Qualifications

• License to practice physical therapy (Montana license or multi-state compact) within 30 days of hire.
• RQI Provider (BLS) within seven days of hire.

Responsibilities

• Develops and revises individualized, goal-directed physical therapy plans based on evaluations.

• Maintains updated plan of care with goals, interventions, and visit frequency.
• Delivers therapy services per established standards and department policies.
• Completes documentation per payer guidelines and Montana practice guidelines.
• Contributes to rehabilitation projects within the department and community.
