Qualifications

• Employee must possess a bachelor’s degree or greater in Physical Therapy from an accredited college or university.
• Must maintain an active, unencumbered license granted from state of employment.

Responsibilities

• Return patient to optimal physical function and facilitate achievement of therapy goals within a SNF setting.
• Educate patients to maintain function after discharge through independent exercise and activity modification.
