Job summary

LeadingAge Virginia is hiring a Physical Therapist for CRCH Rehab in a full-time day shift at 1 Health Cir - Lexington Carilion Medical Center. The PT will assess patients, interpret referrals, and create individualized treatment plans in line with age and diagnosis. Sign-on bonus up to $20,000 applies.

Qualifications include Virginia licensure, AHA BLS within six months, and strong communication and teamwork skills. Will supervise PTAs and collaborate with the care team across settings.

Qualifications

• Graduate of approved school of Physical Therapy; Bachelor's degree and 5 years of experience, OR Master's or Doctorate in Physical Therapy.
• Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy.
• BLS-certified (American Heart Association) within six months of employment.
• Strong interpersonal, communication, customer service and teamwork skills.

Responsibilities

• Provide patient care based on physician referral and age-appropriate assessment.
• Coordinate patient care and clinical activities with the care team.
• Document in the patient’s medical record and complete related forms per policy.
• Supervise PTAs and collaborate with therapy staff and students.
• Participate in performance improvement, program development, and new employee orientation.
