Job summary

Elderwood at Burlington in Burlington, VT is seeking a Full-Time Physical Therapist to join our rehabilitation team and help residents regain strength, mobility, and independence. The PT will evaluate residents, develop treatment plans, deliver skilled therapies, and collaborate with an interdisciplinary team.

State licensure in Vermont (or eligibility) is required, with a strong focus on geriatric and post-acute populations.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current Vermont Physical Therapy license (or eligibility).
• Strong clinical, communication, and documentation skills.
• Passion for working with geriatric and post-acute populations.

Responsibilities

• Evaluate and develop individualized treatment plans for residents.
• Deliver skilled physical therapy services in accordance with physician orders.
• Collaborate with interdisciplinary team members to optimize care plans.
• Monitor, document, and adjust treatments based on resident progress.
• Educate residents and families on therapy goals and outcomes.
