• Assess motor status, mobility, ROM, strength, and rehab potential.
• Direct and implement therapy treatments and plans.
• Instruct patients and caregivers in home care usage of devices.
• Prepare progress summaries and assist in discharge planning.
• Supervise PT aides as required by policy and state rules.
• Collaborate with care teams to identify additional home health needs.
