Job summary

Las Estancias by Pure Health is seeking a dedicated Physical Therapist to provide patient-centered therapy in a post-acute setting. You will evaluate and treat patients, design care plans, supervise PTAs, and document progress while collaborating with an interdisciplinary team.

You will support education of interns and staff, manage caseload, and participate in rounds and care conferences. Weekend and holiday shifts may be required.

Qualifications

• Graduate of an accredited therapy program.
• Valid PT License.
• Preferred: at least 1 year of related experience.

Responsibilities

• Provides direct customer care that meets department and professional standards of practice, including patient evaluation and assessment, development of physical therapy care plan, patient treatment, family/caregiver training and discharge planning.
• Examines, evaluates, and tests customers with impairments, functional limitations, and movement-related conditions; determines diagnosis, prognosis, and plan of therapeutic intervention.
• Designs and modifies therapeutic interventions to alleviate impairments and functional limitations; administers appropriate assessment and treatment techniques.
• Evaluates for, recommends and applies equipment and devices, and trains patients in their use.
• Participates in clinical teaching and mentorship within the team; supports educational needs of team members.
• Performs clinical documentation including assessments, plans, progress notes, discharge summaries, and charges; ensures accuracy and completeness.
• Discharges patients with appropriate discharge planning including equipment procurement and family training; communicates with Recovery Care Coordinator.
• Schedules and manages caseload with adherence to productivity standards and timely documentation and billing.
• Initiates and participates in interdisciplinary care planning; coordinates with community resources to ensure continuity of service.
• Identifies equipment repair or upgrade needs and contributes to selection of equipment and supplies.
• Attends and participates in in-service training.
