Qualifications

• Graduate from an accredited physical therapy program.

Responsibilities

• Patient assessment and ongoing evaluation of treatment programs, goals and discharge plans.
• Establish a plan of care with patient/family members and discharge goals.
• Implement the physical therapy treatment plan and provide patient/family education.
• Document evaluations and plans of care per departmental guidelines.
• Coordinate referrals and discharge instructions, and assist with community resources as appropriate.
