Job summary

Elara Caring is seeking a Physical Therapist Assistant to support home-based patient care across NH. You will work under a licensed Physical Therapist to follow established plans of care and help patients improve mobility and independence.

The role requires an unrestricted PTA license in the state, graduation from an accredited PTA program, and the ability to assist with patient transfers and lifting as needed. A driver’s license or reliable transportation is required.

Qualifications

• Current, unrestricted Physical Therapist Assistant license in state of practice.
• Six or more months of PTA experience preferred.

Responsibilities

• Provide physical therapy services under the supervision of a licensed Physical Therapist.
• Assist patients in improving mobility, strength, function, and independence.
• Monitor progress and communicate changes to the supervising PT.
• Educate patients and caregivers on home exercise programs.
• Teach proper use of assistive devices (canes, walkers, wheelchairs, prosthetics, orthotics).
• Deliver safe, high-quality care per physician orders and state guidelines.
