Job summary

One Home Health is seeking a part-time Physical Therapist to join our home health team serving Valparaiso, IN and surrounding areas. You will perform evaluations, implement care plans, and collaborate with physicians and care teams to deliver patient-centered care.

The role requires an accredited PT program graduate with a current license, CPR certification, and at least 2 years of healthcare experience (home health preferred). A valid driver's license is essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• Minimum of 2 years healthcare experience; home care experience preferred.
• Current CPR certification.
• Strong verbal and written communication skills.
• Ability to work independently with minimal supervision.
• Valid driver’s license with reliable transportation.

Responsibilities

• Perform PT evaluations, treatments, and ongoing assessments per plans of care.
• Complete initial and comprehensive assessments (including OASIS) within 48 hours of referral.
• Develop and update individualized care plans based on patient needs.
• Document evaluations, treatments, and outcomes accurately and timely.
• Communicate patient status and changes to physicians and care teams.
• Supervise and support PTAs and home health aides as required.
