Job summary

One Home Health LLC is seeking an Experienced Home Health Physical Therapist to provide in-home evaluations and therapy across Porter County and surrounding Northwest Indiana communities. The role emphasizes mobility, balance, strength, and independence in patients' homes.

The position is Part-Time/PRN with flexible scheduling and strong clinical leadership. Candidates should have an active PT license in Illinois and be comfortable working in home health settings with supportive

Qualifications

Active Physical Therapist license (IL)
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Valid driver's license and reliable transportation
Excellent communication, organization and time-management skills
Ability to work independently in a home health setting

Responsibilities

Perform comprehensive home-based PT evaluations.
Develop and implement individualized treatment plans to improve mobility, strength, balance and functional independence.
Provide skilled PT interventions according to physician orders and care plans.
Assess home environments and recommend safety modifications and assistive devices.
Educate patients and caregivers on exercises, injury prevention, and home safety.
Collaborate with interdisciplinary care team including physicians, nurses, SLPs, OT.
