Job summary

Qualifications

• Current license to practice as a Physical Therapist Assistant in Indiana.
• Graduate of an accredited physical therapist assistant program approved by the American Physical Therapy Association.
• Minimum of six months experience as a Physical Therapist Assistant.

Responsibilities

• Provide physical therapy services in accordance with the physician’s Plan of Care.
• Complete visits and documentation accurately and within established timelines.
• Monitor patient progress and report changes in condition or schedule.

Skills

Demonstrated communication and interpersonal skills

Licensed driver with reliable transportation
