Responsibilities

• Examines and evaluates patients.
• Establishes diagnosis, prognosis, and individualized plans of care.
• Implements treatment interventions and coordinates rehabilitation plans.
• Completes documentation and charges accurately and timely, in compliance with department policy.
• Communicates discharge and home needs and resources.
• Educates care givers and patients.
• Seeks additional guidance when necessary to assure safe and effective patient care.
• Participates in departmental quality improvement initiatives.
