Job summary

Manna Post Acute in Pickens, SC is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team on a full-time basis. You will evaluate residents, develop individualized plans, and provide interventions to maximize independence, safety, and quality of life.

Responsibilities include monitoring progress, communicating with physicians and staff, documenting per guidelines, and contributing to discharge planning. Licensure is required and PCC/Casamba experience is a plus.

Qualifications

• Bachelor's degree in Occupational Therapy is required.
• SC license in good standing is required.
• Experience in skilled nursing/rehabilitation preferred.
• Experience with PCC and Casamba is preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services per physician orders and goals.
• Monitor resident progress and adjust treatment as needed.
• Maintain accurate and timely documentation per regulations and payer requirements.
• Participate in care planning meetings and discharge planning activities.
• Educate residents, families, and caregivers on treatment goals and home programs.
• Ensure compliance with state acts and facility policies.
