Responsibilities

• Perform accurate and timely evaluations for new patients referred.
• Design an appropriate plan of care with measurable goals.
• Document services provided and maintain patient records.
• Re-evaluate and adjust treatment plans as needed.
• Communicate with physicians and staff to coordinate care.
• Supervise assistants and rehabilitation techs as appropriate.
• Participate in discharge planning and interdisciplinary meetings.
